Manitoba Possible’s Children’s Service Coordinators work closely with families to identify strengths, needs, and goals, assisting families understand and access services and resources available to them in the following ways:
service navigation and facilitating referrals
advocating within various systems like healthcare and education
education on resources available to families
transition planning to daycare, school, and adulthood
coordinating care through collaboration with other professionals like teachers and therapists.
who is eligible.
Children under the age of 18, who live in Manitoba with their birth, adoptive or extended families. Eligible children must also have a medical diagnosis confirming one or more of the following conditions:
permanent physical disability
seizure disorder
brain injury
Deaf/hard of hearing
Some children may not be eligible for Children’s Service Coordination through Manitoba Possible; however, may be eligible for a program through another designated agency:
learning disabilities or attention deficit disorders: Learning Disability Association of Manitoba
autism spectrum disorder: St. Amant
intellectual disability or nonphysical developmental delay: Children’s disABILITY Services
blind or low vision: CNIB
children whose primary residence is on reserve are supported by their community’s Jordan’s Principle service coordinator
children who are in the guardianship of Child and Family Services (CFS) receive case management through their CFS worker.
If your child has multiple diagnoses and you are unsure which program is appropriate, please contact us.
how to apply.
You may be referred to the program by medical professionals, schools, or other professionals working with the child. You can also apply to the program directly by submitting the referral form, along with diagnostic assessment or medical report, to either Manitoba Possible’s Winnipeg or Rural/Northern office.
Please note that you need written confirmation of the child's diagnosis to be eligible.
